    - p. 6, 7: statements regarding "non-blank lines" have been added
    - p. 7: rules for normalization have been clarified treatment of S-parameters
    - p. 7: the word "immediately" has been removed
    - p. 20: multiplication symbol removed, to change equation to 2R 
    - p. 20: multiplication symbol removed from equations (e) through (h)
    - p. 28: multiplication symbol removed, to change equation to 2r0 
          
      Date of the document is Apr. 8, 2009.
